There were no receipt records available to verify that the authorized models of Ir-192 and Cs-137 sources were the types being utilized for patient treatments.

The Licensee failed to provide documentation which could verify and uniquely identify the Ir-192 seeds contained in ribbon as Best Industries, Model 81-01; or Amersham, Model ICW 100 as required by license.
The Licensee failed to provide documentation which could verify and uniquely identify the Cs-137 sources being used in brachytherapy treatments as 3M, 6D6C sources as required by license.

The Licensee failed to maintain documentation to verify that survey instruments used for conducting surveys of patients receiving Ir-192 and Cs-137 sealed source therapy had been calibrated at yearly intervals.
