Unwanted in Radioactive Materials Inspection

  1. Deviation from approved operating and safety procedures;
    inadequate safety procedures; and/or use of procedures
    not approved by Agency.
  2. Radiation surveys (instrument, wipes, or environmental
    sampling) was not performed as required; and/or survey(s)
    were not valid due to use of uncalibrated
    instrumentation.
  3. Personnel monitoring records were absent or incomplete
    for time periods or in content; and/or missing data was
    not accounted for by recording an estimate.
  4. Radioactive sealed sources were not leak tested, not
    tested on required interval, and/or procedure/person
    performing the tests were not authorized.
  5. Radiation detection/measurement instruments were not
    available or not used; and/or those used were not
    calibrated , not calibrated on schedule, and/or not
    calibrated by authorized/required procedure. (This
    includes pocket dosimeters).
  6. Physical inventories were not performed; use logs were
    not maintained; and/or inventory records/use logs were
    absent, incomplete, inaccurate, or incorrect.
  7. Training of industrial radiographic personnel (trainees,
    radiographers, trainers) or well logging personnel
    (supervisors, assistants) was not provided, inadequate,
    unauthorized, and/or training records were not
    maintained. (Included no valid I.D. Card)
  8. Inspection and maintenance of radiation source, transport
    containers, source holders, and related equipment were
    not performed and/or recorded, and/or not performed on
    required time interval.
  9. Radiation source receipt/transfer records were not
    available, incomplete, or incorrect.
  10. Persons using radiation sources were not authorized by
    license, properly trained, or certified.
  11. TRC Form 12-1, Notice to Employees, was not posted, was
    not posted in a sufficient number of places, and/or was
    not readable.
  12. Documents were not available for inspection. This
    includes any document required by the regulations, permit
    conditions, or application, to be maintained for
    inspection.
  13. Possession/use of radioactive materials without a
    license.
  14. Use or storage at a location not authorized by license;
    unauthorized field use; and/or moving from or vacating an
    authorized location without notice to the Agency.
