Unwanted in Mammography Inspection
- Mammography quality control, medical physicist
evaluations, supervising physician review of quality
control, and/or phantom techniques nor patient techniques
were not performed. - Mammography AEC constant density or mammography kVp was
inaccurate. - Mammography quality control failed and/or phantom failed
and did not cease performing mammography. - Technique chart was incomplete.
- Records were not available for protective device test,
machine compliance test, instrument calibration,
mammography quality assurance/quality control, and/or
mammography annual physicist evaluation. - Regulations, operating and safety procedures,
certification, correspondence, and/or radiation
protection program were not available and/or Radiation
Safety Officer quarterly audits were not performed. - Mammography quality control corrective actions were not
performed. - Values on the technique chart were not used clinically.
- Certification was not available, not current, not
complete or had expired.
